• $180 – Routine/Follow-up visits/Medication Management
• $225 – Medical Cannabis Evaluation
• $100 per occurrence – Paperwork Completion for Temporary Disability
• $100 per request- Letter request fee for work, SSI benefits, FMLA, school
• $50 per form – Formula Form
